Mumbai, (Maharashtra) [India]: A cardiac event that once belonged almost exclusively to a patient’s fifties or sixties is now showing up routinely in men barely past their mid-thirties. In a trend that Interventional Cardiologist Dr. Kiran Narang says is no longer worth flagging as an anomaly, men in their mid-thirties to mid-forties, many of them fit-looking tech and finance professionals with no obvious red flags, are increasingly arriving at his catheterisation lab with blocked coronary arteries. For a growing number of them, the first symptom is the heart attack itself.

Widely regarded as a leading Interventional Cardiologist in Mumbai, India, Dr. Kiran Narang has performed close to 5,000 angiograms and around 1,500 angioplasties over more than a decade of clinical practice, and completed his cardiology residency before the age of 30. His caseload, he says, gives him a front-row view of exactly how far the average age of the Indian cardiac patient has been sliding downward. “Ten years ago, a man in his mid-thirties on my table for an angioplasty was the exception I’d remember for weeks,” he said. “Today, it’s simply Tuesday.”

National data has been pointing to this shift for years. Insurance claims for cardiovascular disease among Indians aged 35 and below rose sharply even a decade ago, and cardiologists across the country now routinely report treating heart attack patients in their late twenties. India already accounts for a disproportionate share of the world’s heart attacks in men over 35, and Indians as a population tend to develop coronary artery disease five to ten years earlier than their counterparts in the West, even at body weights and waist sizes that wouldn’t raise concern on a standard chart. Dr. Narang points to South Asian genetics, combined with a demanding modern urban lifestyle, as a particularly unforgiving mix.

“South Asian men carry a genetic tendency toward higher LDL cholesterol and more abdominal fat, even when they look lean, what we call the ‘thin-fat’ body type,” said Dr. Kiran Narang. “Layer on eighty-hour work weeks, deadline stress, broken sleep, and a diet built around refined carbohydrates and late-night takeout, and you have a coronary artery ageing far faster than the man attached to it. The heart doesn’t care what your LinkedIn profile says about how well your career is going.”

One of the most dangerous misconceptions Dr. Narang encounters in his clinic is the belief that a regular gym habit or a marathon medal rules out cardiac risk. Exercise capacity and coronary artery health are not the same thing, and a man who runs five kilometres every morning can still be carrying dangerous plaque if his cholesterol, blood sugar, and blood pressure have never been screened. Silent risk factors such as undiagnosed hypertension, pre-diabetes, and high lipoprotein(a) levels can build for years without a single symptom, quietly narrowing the arteries until a clot finally closes them off, which is why so many of his patients arrive with no warning at all, their first symptom severe enough to require emergency angioplasty.

“I tell every man who walks into my clinic after thirty-five the same thing,” said Dr. Narang. “Don’t wait for a symptom to send you to a cardiologist, because for a growing number of my patients, the first symptom is the heart attack. A basic lipid profile, an ECG, and a blood pressure check once a year is a small ask against the alternative. A stent solves the blockage in front of me, but it doesn’t solve the lifestyle that put it there. My job is to make sure a thirty-six-year-old doesn’t leave my lab having only bought himself a few more years instead of his health back.”

At his clinic in Marol, Andheri East, Dr. Narang’s approach for this age group centres on catching risk early rather than treating damage late: ECG, 2D echocardiography, and coronary angiography where indicated, using the radial, wrist-based approach wherever possible to shorten recovery time, followed by structured, long-term management of blood pressure, blood sugar, and cholesterol rather than a one-time consultation. His message to men in this bracket is direct: chest pain, breathlessness during routine activity, palpitations, unexplained fatigue, or a family history of premature heart disease should never be dismissed as “just stress” or “just work pressure.” Early screening in one’s thirties, he says, is no longer precautionary; for a growing share of Indian men, it is essential.
